When number of employees are between 1 and 5 at the time of application.
When number of employees are between 6 and 10 at the time of application.
When number of employees are between 11 and 20 at the time of application.
When number of employees are between 21 and 35 at the time of application.
When number of employees are between 36 and 50 at the time of application.
When number of employees are between 51 and 100 at the time of application.
When number of employees are between 101 and 250 at the time of application.
When number of employees are between 251 and 500 at the time of application.
When number of employees are 501 or more at the time of application.

Independent Agents, etc.
501c (3) NonProfit: $225.00
Charitable organizations with less than 5 full-time employees.
Home Based Business: $275.00

One Time Setup Fee: $50.00
This $50 fee is only applicable for new members.
